Immunization Requirements

A state public health law requires that the immunization portion of the form be completed and signed by a physician and that all updates of necessary immunizations be noted. The law does not apply to students born before 1957. Students will not be permitted to attend classes or move into the residence halls until this requirement is met.”

Students may obtain their immunization records from their physician, high school, or university/college previously attended. If immunization records are not available then students must receive the titer blood test which shows proof of immunity to MMR and show proof of the DT booster received within the last 10 years.

**The new Illinois state law now requires proof of a meningitis acwy immunization on or after the age of 16.  If the first meningisitis shot was received prior to age 16 a booster is required.
